Job Description

The Case Manager will provide a collaborative and coordinated approach to the delivery of health and social services that link patients with appropriate services to address specific needs and achieve treatment goals.

Responsibilities:

Responsible for providing individual case management for clients in outpatient treatment services program.

Assist clients with applying for and maintaining health and public benefits.

Conduct daily case management sessions with clients

Documents services of patient progress toward self-management. Completes documentation within 24 hours of providing the services

Links clients to community resources and services to maximize independence and support recovery goals, including referrals to local food banks and/or community churches for groceries and meals; clothing assistance; transportation services; vocational services; support for employment; and education.

Facilitate, when necessary, transitions in SUD levels of care including initiating referrals to the next level of care, and coordinating with and forwarding necessary documentation to the accepting treatment agency.

Verify appointments including DCFS court dates and hearings, Probation/Parole, and Medical Appointments.

·Coordinate care with physical health, physical exams for clients to ensure requirements for DMC are met within 30 days, community health clinics and providers, and mental health providers to ensure a coordinated approach to whole-person health service delivery.

Coordinate with state and County entities (DPSS, DCFS, Probation, Superior Courts, Housing Providers, etc.) to ensure the social aspects of health and well-being are being coordinated with health services.

Prepares progress reports for court, social workers, etc.

Advocate on behalf of the client with health/social service providers, County and community partners, and others (such as officials at schools, juvenile or adult court hearings and/or meetings with corrections staff, and Student Attendance Review Boards or other school-related hearings) in the best interests of clients promptly.

Participates in Case Conference Status Meetings
