Job Description
As a member of Mariposa Heritage House and the Drug Court Multi-disciplinary team, the Case Manager works with clients in the Adult Drug Court (Phases 1-4) and will provide Case management to individuals under community corrections supervision. Case management will allow a DC participant a single point of contact within the system
that will help coordinate care and services. This position will link participants with resources, coordinate and ensure continuity of care for the client and provide advocacy and open-communication among the MDT. The Drug Court Case Manager will take on additional tasks that go beyond the ‘traditional “service broker” role and will work in partnership with Court, SUD treatment providers, Probation, Peer Counselors and other providers in and outside of Mariposa County. This position reports to the Program Coordinator
Role
This role will combine both a ‘strengths-based” and “assertive” case management approach and will emphasize an unconditional positive regard for the clients, while working to discover for themselves their “inner strengths” and “resources”. While this position is unlikely to evaluate programs, it will assist the Program Coordinator with data collection.
This position will carry out it’s duties in Mariposa and North County (Hornitos, Greeley Hill, Don Pedro, Coulterville). Degree in Social Work or related field, desired. Work experience may be considered equivalent to a degree. Prefer two or more years of case management experience in a non-profit or other setting where the target population served includes individuals involved with the criminal justice system, substance use disorders and experiencing houselessness. This is a full-time, 40-hours per week position. $19.31-$20.58 per hour with benefits.
